To get equipment for personal use, there’s an easy three-step application process.This includes obtaining the signature of a doctor or other medical professional. Once the Certification Form is complete, mail or FAX your completed form to the State of California CTAP Contact Center or bring it to one of the State of California CTAP Service Centers located throughout California.
Once your Certification Form is approved, you will receive a letter or call. CTAP will contact low-vision and blind customers by phone before sending a letter. The letter or call will instruct you to either get in touch with the State of California CTAP Contact Center or visit a CTAP Service Center. You will be assessed for the best equipment and services to meet your needs. Additional training is also available by getting in touch with the State of California CTAP Contact Center or visiting a CTAP Service Center.
